Dear Valued Patient,
We are writing to share an important update regarding Montana Stillwater Health and your care.
After heartfelt consideration, Montana Stillwater Health will be scaling back and transitioning the practice to exclusively care for patients enrolled in the GUIDE Program (Guiding an Improved Dementia Experience). After March 31st, 2026, our clinic space will be closed, and moving forward services will no longer be offered to patients who are not enrolled in GUIDE. To be eligible for GUIDE, you must have both a diagnosis of dementia and traditional Medicare A/B.
This decision was not made lightly. It is with sincere gratitude that we extend our appreciation to the patients, families, and loved ones we have had the honor of caring for over the years. Knowing this transition will impact each one of you, we have outlined important information and next steps below to ensure a smooth transition and adequate continuity of care.

If you already have an appointment scheduled prior to March 31st, 2026, this appointment will remain unchanged. If you would like to make an appointment with the clinic, please call 406-272-3746 to schedule.

If you are already enrolled in the GUIDE Program and would like to continue seeing Dr. Haseman, no action is required.

If you believe you are eligible for the GUIDE Program and are interested in enrolling, please call 406-272-3746 to inquire about scheduling an appointment for GUIDE enrollment. We will also be reaching out to eligible patients who already have appointments scheduled with Montana Stillwater Health to confirm their appointment and intent to enroll in the GUIDE program.

Alaina Cole, APRN will be transitioning her practice to Providence Grant Creek Family Medicine Clinic effective May 4th, 2026. If you are a patient of Alaina Cole’s, or would like to establish care with this provider, please call 406-327-1850 to schedule a clinic appointment with her at Grant Creek Family Medicine. She will no longer be offering home-based primary care.

For patients currently receiving home visits through our Home-Based Primary Care services, you can continue to receive primary care services in your home through Big Sky Palliative Care. They will now be offering home-based primary care services for former Montana Stillwater Health patients who require or prefer primary care visits in their home. Please call 406-543-4408 to schedule an establish care appointment with Big Sky Palliative Care.

If you are not eligible for the GUIDE program with Dr. Haseman, we strongly encourage you to establish care with another primary care or geriatric provider as soon as possible. We understand that transitions in care can be challenging, and we are committed to supporting you during this time. Prior to April 30th, 2026, we can assist with final prescription refill requests. Please call to request your medical records be transferred to the appropriate care teams.
